Abstract
Failure to hold trainees and PDs accountable to ensuring competence places patients and society at risk. When residents and fellows are identified as not meeting program expectations, program directors (PDs) are responsible to follow established processes for remediation and disciplinary actions if needed. Institutional officials are required to ensure that all actions are transparent and compliant with program and institutional policies. Sponsoring institutions (SIs) for graduate medical education (GME) training programs in the USA and Canada that are accredited by the Accreditation Council on Graduate Medical Education (ACGME) are required to identify a Designated Institutional Official (DIO) to support all aspects of GME in those SIs. The organizational structures of SIs vary widely, but all are committed to providing support for the work of the DIO. This work can be particularly challenging when residents and fellows struggle to perform effectively and require remediation. Delays in training, failures to progress, probation, and dismissal all have significant consequences for the learner, including a potential inability to become licensed to practice medicine. The role of GME trainees as both learners and employees creates unique challenges with regard to the process of managing remediation and its subsequent outcomes. Using the representative literature and their collective experiences, they describe some unique aspects of remediation in GME and the issues that may arise for trainees, PDs, administration, and institution, as well as offer best practices in how to navigate these complex situations.
