abstract = "The occurrence of the episodes of vasodilatory hypotension can be a life-threatening manifestation of systemic mastocytosis. This article describes the reversal by epinephrine of episodes of severe hypotension in two hospitalized patients with mastocytosis. Recognition of the efficacy of epinephrine in hypotension associated with mastocytosis can be important when other methods fail to restore hemodynamic stability.",

(.I ALLERGY CLIN IMMUNOL 71: 189, 1983. ) Mastocytosis is a disorder characterized by an abnormal proliferation of mast cells in any of a variety of tissues, including skin, bone marrow, liver, spleen, lymph nodes, and the gastrointestinal tract. Symptoms and findings may include episodic flushing, tachycardia, pruritus, headache, abdominal pain, diarrhea, and syncope. Unpredictable episodes of profound hypotension are a recognized feature of the disorderle5 and are presumably caused by the release of vasodepressor mast cell products, including histamine{\textquoteright} and PGD,.” Such attacks may be fatal despite treatment with antihistamines, saline, corticosteroids, and pressor agents.3, s In some patients with mastocytosis, attacks may be provoked by physical exertion,” alcohol,6 narcotics ,7 or aspirin. 8 The recent recognition that mastocytosis is associ-ated with characteristic urine biochemical abnormalities, including overexcretion of histamine2 and of a metabolite of PGD,,S has facilitated the diagnosis of this disorder. The chronic manifestations of mastocy-tosis have been reported to be ameliorated by a variety of specific therapies designed to suppress the synthesis,;{\textquoteright} release,” or actions”. lo of mast cell mediators.
