Abstract
The patient that presents with a chronically deficient extensor mechanism (EM) is one of the most challenging clinical problems in association with TKA revision surgery. We define a deficiency as any extensor lag >30 degrees, in association with symptoms of instability as a result of a dysfunctional or disrupted EM. In our experience, this problem occurs infrequently, in the range of 0.1 to 2.5% of all primary and revision TKA procedures. Attempts at primary repair of the EM are unsuccessful in this chronic patient group, as the disruption typically has been present for >6 months. 1.
