Endovascular Management of Aortic Arch Pathology

Abstract

Purpose of Review: The development and use endovascular techniques for treatment of aortic arch pathology has rapidly accelerated in recent years. This chapter presents current methods and devices in-trial for endovascular repairs extending to zone 0 and zone 1. Recent Findings: The approval of the Thoracic Branched Endograft (TBE, GORE) by the FDA in 2022 has greatly increased options endovascular aortic arch repair in conjunction with cervical debranching techniques. Additional trials are ongoing for multibranch and zone 0 devices, with elevated stroke rate identified as a significant perioperative risk. Summary: Endovascular techniques for treating the aortic arch are becoming increasingly popularized for treatment in the appropriate patient population, with additional multibranch and zone 0 devices under active investigation.
