Abstract

The hypothesis that cervical cancer tumor delineation is superior using diffusion weighted imaging (DWI) compared to the corresponding apparent diffusion coefficient (ADC) maps was tested. A radiation oncologist analyzed brachytherapy planning structural MRIs, diffusion weighted MRIs (DW-MRIs), and ADC maps for 19 patients with locally advanced cervical cancer. Pre-treatment MRIs were analyzed separately from post-implant brachytherapy planning MRIs to account for differences in pulse sequences and artifacts associated with the titanium tandem and ovoid implants. DWIs (b = 800 mm2/s) had significantly higher mean contrast ratios (CRs) pre-treatment (0.72 ± 0.34 vs. 0.24 ± 0.15) and post-implant (0.70 ± 0.36 vs. 0.17 ± 0.16) compared to ADCs. For pre-treatment DWIs, mean tumor and uterus signal-to-noise ratios (SNRs) were 54.90 ± 20.30 and 32.36 ± 11.82, respectively. For post-implant DWIs, mean tumor and uterus SNRs were 28.51 ± 10.57 and 16.93 ± 5.13, respectively. Mean contrast-to-noise ratio (CNRs) were 22.54 ± 10.98 pre-treatment and 11.58 ± 6.98 post-implant. DWIs had significantly higher CRs between tumor and uterus tissues compared to ADC maps.
